SILVER ACCREDITATION

The trust/health board/ care provider has had Bronze “Weigh to Go” accreditation for 12 months

There is clear evidence that Bronze accreditation is maintained with the following additional criteria met:

  • If the EPR is the primary record, appropriate specialist growth charts are available on the electronic system
  • Appropriate weighing and measuring equipment (including Wheelchair scales/hoists and appropriate length mats) available in every clinical setting where children are seen
  • The trust/ health board has access to equipment required for detailed and more specialist nutritional assessment (callipers, etc.)
  • There is a written care pathway for management of growth and nutritional difficulties in all children, including specific pathways for vulnerable groups including disabled children, looked after children and those with neurodevelopmental diversity
  • Auditing compliance with growth monitoring for all children is part of trust/health board audit plan
  • There are clear plans for the trust/health board to progress to Gold accreditation
